In order for AAG Health partner physicians to prescribe these hormone medications they must establish clinical need and medical necessity, have a direct professional doctor-patient relationship, and make a diagnosis for: Adult Onset Growth Hormone Deficiency Syndrome; Hypogonadism / Andropause; Menopause; Hypothyroidism; Low Testosterone; etc.

Medical Evaluation

After enrollment, you will be required to go through the AAG Health Medical Evaluation which includes detailed lab testing, physical exam, and medical consultation to determine if you qualify for hormone therapy. We want to make sure that you do not have any underlying medical conditions and actually require hormone therapy based on medical necessity. Hormone therapy is not for everyone.

* In addition to the above requirements, prospective female hormone therapy patients must also provide results of a recent Pap Smear and Mammogram.

Final Medical Assessment

After your medical file is complete you are ready for your final medical assessment. The AAG Health Physician (or PA-C) will discuss with you personally your symptoms, problems, medical history, and if indicated - will make a clinical diagnosis of: Adult Onset Growth Hormone Deficiency Syndrome (AGHD); Hypogonadism / Andropause (Male Menopause Symptoms); Menopause; Hypothyroidism; Low Growth Hormone, Low Testosterone (Low T), etc.
Once the AAG Health Physician (or PA-C) has approved you for hormone therapy he will provide you with your individualized dosage protocol and details of your hormone therapy program.

Delivery Of Your Hormone Therapy Program

You can now come to the AAG Health Clinic office to pick up your Hormone medications and other therapy supplies, or it will be delivered to you via overnight courier. You will also receive a print out of your hormone therapy instructions and exact dosage protocol.
Your AAG Health Patient Care Specialist and/or AAG Physician (or PA-C) will discuss the hormone medications and go over your individualized dosage protocol and instructions in detail.

To make sure your hormone therapy program is a success you are required to comply with our therapy follow-up, monitoring and re-evaluation protocols.
You must contact us 4-6 weeks into the start of your therapy program (and every 6 months thereafter) to discuss your progress and arrange for follow-up blood testing and/or office visits/consultations.
Your AAG Health Patient Care Specialist and the AAG Physician (or PA-C) will be at your disposal for office visits/consultations during normal clinic hours (Monday thru Friday: 9:00 am to 6:00 pm EST).
Please take note that office visits/consultations with the AAG Physician (or PA-C) are available by request only.
